What to do in case of hospitalization?
Notify the company:
- by telephone on 016 24 24 24 (call centre hospitalisation): every working day from 8h to 12h and from 13h to 17h.
If you have an AssurCard, keep the patient's card with you and provide the card number. If you are not in possession of an Assurcard, have your attending physician fill in the form "declaration hospitalisation". You can find this form on our website under the heading "forms".
